Before you start

Complete this form to:

  • get a letter confirming your National Insurance number
  • report a change of details

You may need to send up to 2 of the following documents to confirm your identity to HMRC:

  • birth certificate
  • passport
  • full or provisional driving licence (plastic card)
  • adoption certificate
  • home office or travel document
  • work permit
  • certificate of naturalisation
  • marriage or civil partnership certificate
  • certificate of service in Her Majesty's Forces or merchant navy
  • identity or medical card

The letter HMRC send you can be used as proof of your National Insurance number.

If you do not have a National Insurance number

Do not complete this form if you've never had a National Insurance number before. Instead you should apply for National Insurance number online.

Eligibility

You must be over the age of 16.

You must already have a National Insurance number even though you may have lost or misplaced it.
